After you complete the application process through the Student Affairs Office, the Neurological Surgery Department requires that you email a copy of the following to the Neurological Surgery Program Coordinator, Alicia Aracena:

  • Your CV
  • A photograph
  • A letter of recommendation (letter not required for Columbia University Students)
